The hero's personal card contains the following basic information:

  2. Passport and biographical information - full name; dates of birth and death; Place of Birth; education; pre-war profession, etc. ;
  3. Military biographical information - date and place of conscription; military rank; position; military specialty, etc. ;
  4. Combat path - military company, front, army, branch of service, key battles, wounds and concussions, last duty station, etc. ;
  5. A brief description of the heroic deeds and / or military merit of the participant in the war, set out in the War Participant's Award Sheets;
  6. Awards - frontline, jubilee, foreign countries and others; archival information and documents confirming them ;
  7. Date and reason for withdrawal / exclusion from the lists of a military unit with differentiation by categories of withdrawal - killed, wounded, captured, missing, returned after the end of the war, demobilized, another reason - documents confirming the reason for the withdrawal;
  8. Date of release from captivity / concentration camp and related documents;
  9. Date of death; country and place of burial, supporting documents;
  10. Details of the source of information - name, fund number, inventory number, case number, type, number and date of the report, part name, etc. ;
  11. Recruitment documents; notifications; letters from the front and to the front; personal photos - front-line, post-war and / or front-line monument, etc. ;
